What Makes a Feedback Form Work?

The best feedback forms share three characteristics: they're short (under 5 questions for most use cases), they ask about specific experiences rather than vague generalities, and they make it easy to respond — with a mix of quick-click options and one optional open-ended question.

Keep it under 5 questions — every additional question reduces completion rate by ~10%.

Ask about a specific recent experience, not general opinions about your brand.

Use star ratings or 1–5 scales for satisfaction — they're faster to answer than text.
